Register with Carepoint Practice

How to Register as a patient 

You are able to register with Carepoint Practice. Below is our catchment area highlighted in blue. We are currently accepting registrations from patients outside of our catchment area. 

You are able to register with Carepoint Practice online or in person. You are asked to provide identification such as a passport or drivers license if you are registering online. If you do not have scanned identification, you are able to complete the registration form online and attend reception to show identification.  

We will be unable to process your registration without identification. If you do not have identification, please attend reception to register.

For online registration, the registration time is approximately 5 working days. For registrations via the pack, the registration time is approximately 14 working days.

Regular patients

You are required to fill out both GMS1 forms and a medical questionnaire (Adult or Child).

Please notify us of any changes as soon as possible so that we can amend our records accordingly. Click here to download our change of address form to complete and bring to reception.
