How to Register as a patient 

If you wish to register with us, please bring in your medical card or ask the receptionist for a Registration Form and a Medical Questionnaire. Alternatively, please download one of the Registration form and Medical Questionnaires from below. 

1. Regular Registration - Please download the registration Form (Purple) and either Adult or Child Medical Questionnaire.  

2. Temporary Registration - Please download the Temporary Patient Registration from (Blue) and Medical Questionnaire. 

3. Catchment Area - Please see the following catchment area for our patients.

The map is a guide only. We are currently able to accept patients outside the catchment area/boundary 

 

 

 

 

 

 

 

New Patients  

All new patients wishing to register with the practice may be asked to provide proof of identity and residency when submitting an application form. For proof of identity we accept birth certificates or a valid passport. Proof of residency can be in the form of an up to date utility bill, bank statement, credit card statement, tenancy agreement, solicitor’s letter regarding purchase of property or employment/DWP. However, patients who are unable to provide these identity documents will not be refused registration.

Change of Name/ Address/ Phone Number

Please notify us of any changes as soon as possible so that we can amend our records accordingly. Click here to download our change of address form to complete and bring to reception.
