Rapid Response Team
Provides a Borough wide service for patients over the age of 18 years who will be assessed by an advanced assessment nurse or therapist, depending on the needs of the patient, within 2 hours of referral. The service aims to prevent unnecessary admission to hospital and provides acute treatment in the community.
The team is comprised of advanced assessment nurses, physiotherapists, health care assistants, occupational therapists, a specialist mental health nurse and a part time social worker. Treatment is provided for up to ten days, with appropriate referral for any on-going needs to community based services and or social care, as required.
The service can also refer to a night carer service which is provided for up to 3 nights. The service operates 356 days a year, from 09.00 to 00.30 for assessment and from 08.00 to 21.00 for the provision of care (assistance with activities of daily living.)
