What is a Primary Care Network?

A primary care network is a group of practices who work together to focus on local patient care. They’ve been around since 2019 and 99 per cent of practices across England now work as part of one.

North Connect PCN is one of 6 PCNs in The London Borough of Hillingdon.

The 6 PCNs and their 42 constituent GP Practices are all members of a GP Confederation. The Confederation, a Community Interest Company, promotes collaborative working to deliver place based integrated health services to the population of Hillingdon and its communities.

We work together to share best practice and develop new services for the benefit of all patients regardless of your registered practice. Each practice still maintains their autonomy in providing care for their registered patients and they all have access to the services provided by the PCN.
